First Visit Information

During your first visit to our office, we will ask that you complete paperwork on your medical history (links to all paper work below). If you are transferring from another practice, we can contact your former doctor to obtain copies of your medical records. At your first appointment, your physician will answer questions, discuss any concerns, and perform a physical examination.

Appointments

Please call 717-792-1811 to book your appointment. Most visits for urgent matters are able to be scheduled the same day.

Cancellation / Tardiness Policy

We ask that you call to cancel and reschedule any appointment you cannot make at least 24 hours in advance. There is a $35.00 no-show fee.  If you are more than ten minutes late to your scheduled appointment, you may be asked to reschedule.  

Insurance and Payment Information

We accept most major health insurance plans. You must have valid insurance to be a patient at our office. We do not participate with Medical Assistance or Medicaid. Call us to verify your insurance.

Our fees are based on the complexity of your illness and the services rendered. Co-payments are collected at the time of your visit; we accept cash, check or credit cards.
